Start with the animal you are caring for so the tracker fits the kind of care they need.
Track the health issue that matters most right now, while keeping room on the sheet for their other conditions.
Bring the tracker to the next vet visit so you can spend less time trying to remember and more time talking about what they need.
Built with care
A health concern can have the same name across species, but what is useful to observe may be different. Your Pet’s Health Log lists each concern once, then gives you the version built for your pet.
Focused: each sheet centers one primary health concern so the daily record stays useful.
Whole-picture: every sheet has room to note other health conditions.
Personal: you can optionally add their name and photo before downloading.
Accessible web worksheet: every tailored form can also be filled in online with a keyboard- and screen-reader-friendly worksheet.
Free: every pet health tracker can be used without an account or email address.
Pet health tracker questions
Each printable focuses on one main health concern and gives you room to record symptoms, day-to-day changes, medicines or measurements your veterinary team asked you to track, other health conditions, and questions for the next visit.
Yes. The trackers are designed to keep observations together so you can bring a clearer record to a veterinary appointment. Your veterinary team decides what the observations mean and what care is appropriate.
Because keeping notes for a sick pet should not cost money. The site is simple to host, has no ads or tracking, and is supported by optional tips and the optional paid household binder. Every condition tracker stays free either way.
No. It is your own record of what you notice. Official records and vaccination certificates come from your veterinary practice.
No. Every tracker is free to use without creating an account or giving Your Pet’s Health Log your email address.
No. Optional names and photos stay in your browser and are added to the PDF on your device. Your Pet’s Health Log does not upload them.
No. Your Pet’s Health Log organizes what you observe. It does not diagnose disease, recommend medication doses, set treatment targets, or replace care from a veterinarian experienced with your pet's species.
Who made this?
I built Your Pet’s Health Log after caring for pets of my own and wanting a simpler way to keep useful observations together. I am not a veterinarian; this project organizes what you notice so your veterinary team has a clearer record to work from.
Real person: there is a named builder behind the project.
Clear boundary: record keeping here never replaces veterinary judgment.
Privacy-first: no account wall, advertising tracker, or health-data analytics script.
Care & safety
Your Pet’s Health Log helps you organize what you notice between vet visits. Choosing one primary concern for a tracker does not mean their other conditions are less important. It does not diagnose disease, recommend medication doses, or set medical or husbandry targets. Use the plan given by a veterinarian experienced with their species, and seek prompt veterinary care for urgent or concerning changes.
